The following actions can be performed on the Employee Portal (button to site below):
- Update personal information including address, email and phone number
- Add, delete or change your accounts for direct deposit
- Update your Federal and State tax withholding
- View, download, or print past pay check stubs
- View, download or print past W2s
- View current benefit enrollments
- View, download and print information about your benefit plans (if administered by MidwestHR)
- View, download and print information about the Transamerica 401k plan (if adopted through MidwestHR).
If you prefer to submit a form to make changes (address, direct deposit or taxes), please reach out to your Payroll Specialist at [email protected] or by calling (630) 836-3000. Your Payroll Specialist, as well as your entire MidwestHR Service Team, can be found in the “Contact Us” widget on the Employee Portal.

Please reach out to your HR Specialist if you need information or forms for any of the following actions:
- New Hire Onboarding
- Change Employee Information (title, pay rate, location, department, deductions, or address)
- Expanding or hiring employees in a new state
- Work Related Injury
- Request for a Certificate of Insurance (for policies provided by MidwestHR)
- Performance Appraisals
- Disciplinary Action Form
- Notice of Separation/Termination
